启动任何程序时出现输入/输出错误

我在 Ubuntu 16.04 上启动的每个程序都会产生输入/输出错误：

$ top
bash: /usr/bin/top: Input/output error
$ pkill firefox
bash: /usr/bin/firefox: Input/output error
$ okular
bash: /usr/bin/okular: Input/output error

发生了什么事？无需重启就能修复吗？文件系统损坏时如何备份？
